As an aside, this survey tool is substandard. 1. Scoring 1-5 isn't clear. 2. Submit button is missing.

Company Response: I'm glad everything went great with our tech! In regards to the substandard survey design, 1 is labeled poor and 5 is labeled as great. The submit button is also very large and labeled "Share your experience". I got some input from outside UX contacts and made changes as of today, so the next time it should be a lot nicer. -Dan C
